At Casa Serena Congregate Living, we provide advanced 24-hour skilled nursing care in an intimate six-bed subacute setting designed for individuals with complex medical needs. • 24-hour licensed nursing supervision
• Complex medication management
• IV therapy and infusion management
• Central line and PICC line care
• Advanced wound care and pressure injury management
• Respiratory services, including oxygen therapy management, nebulizer treatments, respiratory monitoring, and coordination with pulmonary providers
• Tracheostomy care (if applicable to your scope)
• Post-hospital recovery and subacute stabilization
• Rehabilitation coordination (physical, occupational, and speech therapy)
• Full assistance with activities of daily living
• Nutritional oversight and specialized dietary support
• Dementia care with close monitoring and redirection
• Onsite hospice services through contracted hospice partners
